Cushing Systems Public Safety Information Management System (PSIMS) is a fully integrated software package that handles everything from the initial 911 call to the release from Jail. The NIBRS compliant product includes Computer-Aided-Dispatch (CAD), Records Management System (RMS), Mobile, Civil and a Jail module.

A recent enhancement to the Jail module includes an evidence disposition form in PSA allows the user to print out an evidence disposition form for one or all pieces of evidence for a case.

Creating the Evidence Disposition Form for all evidence

The evidence form for all pieces of evidence can be found under the evidence tab of Case Edit.

Creating the Evidence Disposition form for individual pieces of evidence

The evidence form for each individual pieces of evidence can be found when clicking on the edit button for a piece of evidence. Next to the Print Label button will be a Forms dropdown. In this dropdown will be the View Evidence Forms button to create the evidence form the piece of evidence.

The Evidence Disposition Form

After clicking the View Evidence Form, the Evidence Disposition Form pdf will open in a new screen. All pieces of evidence will be listed with their descriptions from the case.

The fields for the evidence form property are:

1. Item: The tag number of the evidence.
2. Date Entered: Entry Date
3. Entered By: Custody By
4. Date/Time Received: Custody Date
5. Date/Time Released: Status Date, if status = Released
6. Brand: Brand of the item
7. Model: Model of the Item
8. Type of Loss: Based on NIBRS property loss
9. Serial No.: Serial number of the item
10. Property Type: Property type of the item
11. Quantity: Number of Items
12. Unit: Quantity in Units
13. Value: Value of the Item
14. Date/Time of Loss: Date and Time of loss
15. Date/Time recovered: Date and Time item recovered
16. Location: Location of item
17. Located at the bottom of the form is the Disposition of the evidence. This section contains fields that are filled out by hand and a signature line.
